Mercy Culture Care Ministry Intake Form

Please be informed that Mercy Culture Care is available to members only. Mercy Culture provides care through one-on-one ministry and small group settings. Mercy Culture pastors and lay leaders are not licensed counselors. If additional emotional and spiritual support is needed, we will recommend professional biblical counseling through a number of available resources. If you are in need of marital or pre-marital support, both partners must be members of Mercy Culture and each fill out this form.

Please Note** Pre-marital ministry requires both partners to be Mercy Culture Members for at least 6 months, also serving and tithing. It requires a commitment to attend 6 to 8 pre-marriage counseling sessions. Therefore, pre-marital ministry requests should be submitted at least 6 months before the wedding date. If you do not meet these requirements, then you can submit a general ministry request.
